For hourly Home Healthcare, our Registered Nurse we will conduct an initial assessment to determine the type of home healthcare services that best meet your needs and tastes.
If you are a Medicare beneficiary, our clinical team will consult with your physician to determine the type of skilled services you will need for your specific medical condition.
Medicare services must be provided in your residence, which can be your home, an Assisted Living Facility or an Independent Living Facility.
Private duty services can be provided anywhere you need them.
Services provided under the Medicare benefit are covered at 100%. Prices for private duty services, such as hourly or live-in care, vary depending on the level of service you select. Call 561-819-0460 to learn more.
Activa has some bilingual staff that are fluent in more than one language. If yours is not a language that they are proficient in, they have the ability to use an interpretation service to assist with communication.
